To Obtain a Record

Any individual under the age of 18 cannot obtain a copy of their record. You may order copies of your birth certificate online or by mail.

Fee Schedule

The cost for the certified copies is $20 for the first copy and $3 for each additional copy.

Obtain Records by Mail

In order to obtain a certified copy of a birth record by mail the following steps need to be taken:

1. Send a written letter with the following information:
   1.     Full name on the birth record
   2.     Date of birth
   3.     Mother's maiden name
   4.     Father's name
2. Make sure your written signature is on the request
3. Send a photocopy of your driver's license
4. The cost for the certified copies is $20 for the first copy and $3 for each additional copy.
5. Enclose a check or money order for the number of copies you wish to receive.
6. Enclose a self-addressed stamped envelope for the return of the documents.
7. Send the request to:
